Refreshing modified content

If you're using apache and mod_expires, you can cusomize you TTLs
pretty specifically, per file type, mime and directory. With that you
can settle for a set of compromise TTLs that suit everybody. Than
again, you can use a proxy aware CMS like pressflow.
