 We are seeing very strange behaviour on a site. We are seeing this both
 with the 1.1.2 release and the current code from the 1.2 branch (at
 revision 2633).

 What is happening is that some requests are send directly to the backend
 without being rewritten as configure in vcl_recv. This appears to be
 happening on uncached (passed) requests. Even stranger: these requests are
 visible in the logs of an apache in front of varnish and in the logs of
 the backend server, but not in the varnischncsa or varnishlog output.

 Running varnishd in debug mode (with -d -d) does not show any errors or
 crashes.
