 Currently when varnish serves an error page (404/503, etc...) there is no
 way to determine which varnish server from a load balanced pool issued the
 error. Ideally I would like the ability to serve a custom error page with
 ESI includes but critically I need a way to identify the varnish instance
 which could be resolved just by adding the hostname into the outputted
 html.
