 Backend requests should have their own transaction ID instead of reusing
 that of the client request that triggered them.

 {{{ReqStart}}} and {{{ReqEnd}}} lines should be generated for backend
 requests as well as client requests.

 Every log line should be tagged with the XID of the request it belongs to,
 or 0 if it's not directly related to a request.
