 Random list of things to try for the lack of a better clue:

 Capture a varnishstat snapshot when it fails.

 Do binary search on number of test-files, to see if there is a threshold
 where the problem appears,
 in particular look for effects as working set size hovers around RAM size.

 Try other count/size combinations, 512 files @ 128M, 2048 files @ 32M etc.

 Try binary search on number of clients to see if there is a threshold

 Set very long/short TTL on objects in VCL to see if expiry is involved.

 Disable some CPU cores and see how that changes the picture.
