 For the failing request there is an explicit return (pass) in vcl_recv
 that prevents the default vcl_recv handling from being executed. Does your
 backend application depend on the x-forwarded-for header, perhaps?

 I can't see anything here that indicates that Varnish is at fault. Try
 varnish-misc or the forums for further help. Closing this.
