libvmod-example moved to /varnishcache/

Hi all.

To make collaboration easier, I've moved vmod-example to:

    https://github.com/varnishcache/libvmod-example/

Old location on /varnish/libvmod-example still has some open pull
requests and issues, I'll try to handle those.

I've added a README with the new link.
