I work on a stack which does something similar, and I blogged about it a
while ago: http://ajlanghorn.com/2015/05/varnish-includes/

In your case, you should be able to do copy a good chunk of that, but
change the return to the lookup function for beresp.http.X-Frame-Options =
"value" or something.

Let me know if you'd like me to expand on that.
