 If more than one ESI include directives are placed in a page, the second
 and subsiquent includes will not be cached properly.

 * Running latest varnish trunk[[BR]]
 * The behavior is the same regardless of the size of max-age and the order
 in which includes are placed.[[BR]]
 * The containing document is cached properly, as is the first
 include[[BR]]
 * The ESI objects are inserted, and seem to expire on schedule, but
 subsequent lookups which should hit on the cache entries do not[[BR]]
 * An observation, which could easily be a mis-correlation: second and
 subsequent requests for esi fragments are logged with an 'XID' of '0'. I
 do not know if this is intentional or perhaps indicative of the underlying
 problem.
