 Additional findings.

 * I can see the missed includes expire on time, ONE PER REQUEST. For
 example, on the esi page with two includes, one of which is missed, if I
 load the page ten times in twenty seconds, I will see ten objects expire
 (in addition to the main document and the first include). The objects are
 obviously stored, but still miss on further requests.
 * The object goes to the MISS step in cache_center cnt_lookup() where
 there is a check on sp->obj->busy.
