 Hi,

 Thanks for your feedback. The diagram is meant more to be an aid in
 understanding the flow through Varnish and doesn't necessarily reflect the
 whole situation. The hash_always_miss functionality is an afterthought
 added later, to address a specific situation that the VCL flow couldn't
 address, and that is why it isn't part of the diagram. Varnish version 4
 draws closer, and this functionality will be more clearly defined and part
 of the design process from the get go.

 I am leaving the ticket open for now as a reminder to see if we go back
 and update the diagrams to reflect this (and the accompanying
 hash_ignore_busy).
